A little planning today can bring peace of mind for tomorrow.

This is a free service from Robert Holland Funeral Services. Recording your wishes does not purchase a funeral plan or set aside money towards a funeral. There is no payment or obligation, and you can ask your funeral director to update or remove your record.

Making your wishes known is a thoughtful way to help those closest to you understand what matters most when the time comes.

Whether you have clear ideas or would simply like to record a few personal preferences, this form gives you a place to put them in writing.

There is no obligation to purchase a funeral plan, no payment required and no need to make every decision today. Your wishes can be updated by contacting your chosen funeral director.

  • This records your personal preferences; it is not a prepaid funeral plan.
  • Completing it does not create a financial commitment.
  • Your wishes are guidance for your family and funeral director and may not be legally binding.
  • You can contact the funeral director at any time to see, change or remove your record, or to withdraw your consent.
  • Only authorised staff at this funeral director can see your record, and every time it is opened or downloaded this is logged.
  • We keep your wishes for 5 years at a time. Before then we'll check with you that you still want us to keep them; if you don't, or we can't reach you, we'll delete them.
  • Any health or religious details you choose to add are only used to carry out your wishes.
  • If you name a trusted contact, please let them know you've shared their details with us.

Your information will be handled in accordance with our Privacy Policy. If you're unhappy with how we handle your information, you can complain to the Information Commissioner's Office (ico.org.uk).

If you request an emailed copy, it will contain the details you entered. Email may not be a secure way to store sensitive information.
